Setting the Password for your Form

Password protecting your form is a simple three-step process:

Toggle the "password protection" option in the "Share->Form Settings" menu
Type in the password with which you want to protect your form
(Re)Publish your form

That's it, from the moment you complete step 3 nobody will be able to load your form or survey without first entering your password! The gif below walks you through these three steps.
